[Medical Today] Effects and Precautions of Under-Eye Fat Repositioning

Among cosmetic surgeries for middle-aged individuals, the most preferred eye surgery is lower eyelid fat repositioning. Under-eye darkness, puffiness, wrinkles, and dark circles—major concerns for this demographic—are cited as primary causes that make the face look older and further accentuate the signs of aging.
Because the skin around the eyes is thin and has little fat, it is one of the areas of the face where signs of aging appear most quickly. If the area under the eyes appears sunken and hollow, or if symptoms such as dark circles or bulging under-eye fat become prominent, it is easy to look tired or older. In such cases, one may consider under-eye fat repositioning surgery, which evenly redistributes the fat to change one's appearance.
Looking at the cause of under-eye fat, it is formed when the orbital septum supporting the fat weakens and is pushed forward, causing a bulging phenomenon and the formation of under-eye furrows. To improve this, rather than simply removing the fat, a more definite effect can be achieved by using a "Double Septum Reinforcement Under-Eye Fat Repositioning" procedure. This involves tightening and fixing the orbital septum and internal fat while properly redistributing the fat to address under-eye wrinkles, loss of elasticity, hollowness, or bulging under-eye fat.
Under-eye fat repositioning surgery is a procedure that flattens the area under the eyes without skin incisions. A small opening is created in the conjunctiva on the inner side of the eyelid, and the surgery is performed through this opening. By repositioning bulging fat into sunken grooves, the protruding areas recede, while the hollowed-out areas are filled with volume, resulting in a flattened, elastic under-eye area. This can transform a tired or aged appearance into a bright and youthful look.
Under-eye fat repositioning can not only improve various under-eye issues but also offers a variety of surgical methods. Since individual characteristics—such as the amount and structure of under-eye fat, skin thickness, and degree of sagging—vary from person to person, under-eye fat repositioning should not be performed using a one-size-fits-all approach but rather tailored to the individual's condition, utilizing methods such as incision, partial incision, or laser repositioning.
In addition, since lower eyelid fat repositioning is a scarless surgery with a relatively quick recovery, swelling generally subsides by 70% about two days after the operation, allowing for a quick return to daily life.
Dr. Hong-Jin Kim of 1mm Plastic Surgery Clinic stated, “The procedure may vary depending on the condition of the fat under the eyes,” adding, “Since under-eye surgery is a difficult procedure dealing with a sensitive and complex area, it is advisable to receive an accurate diagnosis and detailed consultation regarding the desired eye condition at a clinic with highly skilled specialists.” He further advised, “As all plastic surgeries can result in side effects such as bleeding or inflammation depending on the individual patient’s condition, it is essential to receive systematic guidance.”
